Nia Ward
Stage coordinator
What happened from your point of view?
I marked the demonstration safe because the performance had to continue.
What should I check independently?
Vera had sent me a warning about the support before I signed the card.
HARD · CASE FILE
Hard · A demonstration changed at the last moment
An understudy is injured after replacing a lead performer. The engineer blames the replacement, while the coordinator signed a safety card despite a warning. Establish the deliberate act, its intended target and the assistance that allowed it to proceed.
Read the case file below, or open the interactive desk to examine records, keep notes and submit your reasoning. These are the case materials, without the solution.
The demonstration support is secured after the injury. The supplied expert report establishes deliberate unsafe interference before sealing; no outside engineering knowledge is required or expected.
Stage coordinator
I marked the demonstration safe because the performance had to continue.
Vera had sent me a warning about the support before I signed the card.
Injured understudy
I accepted the substitute role without being told that any support warning existed.
The coordinator gave me the signed safe-to-use card.
Event medic
I documented Dana’s unexpected withdrawal and treated Ilan’s injury.
The independent inspection supplies the cause; changing a performer does not itself alter the apparatus.
Prop engineer
I prepared the demonstration support to its approved condition. The injury must have come from the late performer change.
The apparatus was sealed before the understudy replaced the lead.
Lead performer
I was the scheduled demonstrator until an illness forced a late withdrawal.
My replacement was agreed only after the apparatus had been sealed.
Read each record on its own terms. A claim made by a person is different from a fact established by an independent record.
Physical record · Stage desk
Dana is the assigned demonstrator when Vera prepares and seals the apparatus. The assignment is fixed in the signed rehearsal sheet then available to the crew.
Physical record · Medical station
The medic’s independently logged withdrawal is unexpected. Ilan replaces Dana only after the final apparatus seal and receives the existing safe-use card.
Physical record · Stage desk
Nia signs the demonstration safe after receiving Vera’s message. She gives Ilan that card without disclosing the warning. The message and signature predate the injury.
Physical record · Stage desk
Vera tells Nia that she has deliberately made Dana’s demonstration unsafe and asks her to suppress the warning. Nia replies that she understands the intended injury and will clear the demonstration anyway.
Physical record · Prop workshop
Vera signs that the inspector’s approved condition remained unchanged. The expert comparison and sealed custody record contradict that declaration.
Physical record · Medical station
Omar’s original note confirms there was no earlier replacement agreement or notice to Vera before the apparatus was sealed. The intended role and actual injured person therefore differ.
Physical record · Prop workshop
An inspector records the approved condition. Vera then has sole supervised preparation access before the final seal and records the altered support as unchanged. No other person accesses its interior before the demonstration.
Physical record · Prop workshop
The support was deliberately left in an unsafe condition before the final numbered seal. The unsafe condition caused the injury. It was not created by substituting a different performer.
